Money that moves while someone is being cared for
Healthcare payments happen at stressful moments, from several payers at once: the family, the insurer, sometimes an employer or a scheme. Getting deposits, approvals and refunds right is as much about trust as about money.

Who pays a healthcare bill
Unlike most businesses, a hospital often has several payers for one bill, each on its own timeline. Keeping them apart on the patient's account is the foundation of everything else.
| Payer | When they pay | What to track |
|---|---|---|
| Patient or family | Deposit at admission; balance at discharge; at the counter for outpatient visits | Deposits, balances, refunds |
| Insurer or its administrator (cashless) | Approval during the stay; settlement later | Approved vs billed vs settled, and deductions |
| Insurer (reimbursement) | To the patient, after discharge; the hospital is paid by the patient | Documents the patient needs from you |
| Employer or corporate tie-up | Against invoices, on agreed terms | Credit limits and overdue invoices |
| Government or other schemes | As the scheme's rules set out | Scheme claims separately from the rest |
Deposits and refunds done well
An admission deposit is the family's money held against a bill that doesn't exist yet. It should be visible on every interim bill, and anything left over should go back at discharge, not weeks later after someone chases it.
Refunds are simplest when they go back the way the deposit came: to the same card or UPI account. That keeps the refund tied to the original payment and avoids collecting bank details from a family at a difficult time.
Receipt for every deposit. Immediately, with the patient's account number.
Deposits on every interim bill. So the family always knows where they stand.
Refund at discharge. Initiated the same day the final bill is settled.
Back to the original method. No new bank details needed.
Insurer settlements: the long tail
For a hospital with many insured patients, insurer money is a large share of revenue and the slowest to reconcile. Claims are settled after discharge, often several at a time, and the amount paid can differ from the amount approved or billed.
Treat every claim as its own item with a status, and match each settlement line to its claim. Differences need a reason recorded, because they decide whether the balance is billed to the patient, disputed with the insurer or written off.

Clinics, labs and pharmacies
Outside hospitals, healthcare payments are smaller and faster, and most of the work is making them easy for patients who are unwell or worried.
Clinics
Consultation fees at the counter by UPI QR or card; prepaid online for teleconsultations, with a clear policy if the doctor can't attend.
Diagnostic labs
Payment at the collection centre, or a link sent with the home-collection booking; refunds when a test can't be performed.
Pharmacies
Counter sales and home delivery, with cash on delivery or a link before dispatch; returns handled against the original payment.
Keeping health information out of payments
Payment records travel further than you might expect: through banks, payment apps, statements and reports. A description that says what a patient was treated for exposes it to all of them. Use invoice or visit numbers in payment descriptions and links, and keep the clinical detail in your own systems.
Health and payment information are personal data under India's data protection law, and your sector may have its own rules. How they apply to you is a question for your adviser.
Diagnoses, test names, procedures or medicines. An invoice or visit number is enough to match the payment.
Estimates before treatment
Many billing disputes start with a surprise at discharge. A written estimate before a planned admission, with what it includes and what could change it, sets expectations for the family and the deposit. Update it when the treatment plan changes, not only at the end.

Healthcare payment questions
How should hospitals handle advance deposits?
Record each deposit against the patient's account, show it on every interim bill, and refund any balance at discharge to the method it came from. Delayed deposit refunds are among the most common complaints families raise.
How are cashless insurance payments reconciled?
The insurer or its third-party administrator approves an amount during the stay and settles the claim later, often in batches covering many patients and sometimes with deductions. Track each claim separately: approved, billed, settled, and the reason for any difference.
What's the best way to collect for home sample collection or home visits?
A payment link sent with the booking confirmation, or a UPI QR the staff member carries. Both avoid cash on visits and give the patient a receipt straight away.
Can patients pay in instalments for large treatments?
Hospitals can offer their own instalment arrangements, and some patients use loans or card EMIs from regulated lenders. If you offer third-party financing, the lender makes the loan and the digital lending rules decide how it's disclosed.
What should never go into a payment description?
Clinical details. Payment references and descriptions travel through banks and apps; use an invoice or visit number, not a diagnosis, test name or procedure.
Does LILPE work with healthcare providers?
